The Audubon Public Library serves as a Passport Acceptance Facility. New and renewal passport forms are available at the library. Gail Richardson is the Passport Agent trained to assist in answering all questions and completing applications for U.S. passports.
U.S. Passport Requirements:
1 - Completed DS-11 (Application) - Use black ink.
2 - Photograph - The Audubon County Treasurer's Office is available to take the photograph. ($10.00 fee)
3 - Two forms of identification are needed 1) Original Birth Certificate and 2) Driver's License (copy of front & back)
4 - Two payments with a personal check or postal money order for the following:
A) U.S. Department of State - ($130.00 - adult book ) or ($30.00 -adult card*) -Personal Check or money order (No Cash or credit card)
OR - U.S. Department of State -($100 - minor book) or ($15 - minor card*)
B) Audubon Public Library - ($35.00)
* The passport card is good only for driving across the border to Canada or Mexico from the U.S.
Minors under the age of 16 must have both parents accompany them. Both parents must also provide a picture ID.
Contact the Library at (712)563-3301 to schedule an appointment. An agent is available Monday through Friday (10 am - 1 pm) or by special appointment.
*Passports are currently taking from 8 - 11 weeks to process from application to receipt of the passport. Passports that are expedited will take from 4 to 6 weeks. This service costs an additional $60.
